✅ To qualify, applicants must provide:

• 🏠 Must have lived in the home and been the owner as of January 1, 2024

• 📄 Copy of 2024 second installment property tax bill

• 🧾 Proof of payment of property taxes
  (Chase Bank receipt or Cook County Treasurer proof of payment)

• 🪪 Copy of driver’s license

• 💧 Copy of most recent paid water bill
  (Not required if living in a condo)

• 🚗 2025 vehicle sticker numbers written on the application
